Job Description:

InterCare Community Health Network is a not-for-profit organization headquartered in Bangor, Michigan. We employ 400 medical and dental health professionals and support personnel at multiple locations. InterCare is a federally qualified health center dedicated to improving individual health and reducing health issues in communities that we serve. We pursue our vision through the delivery of a broad range of primary care services and by promoting open access for all. InterCare's main focus is on disease prevention, wellness, and regular and timely treatment of chronic conditions. We pride ourselves on excellent communications with our patients, and on a strong health education focus that helps our patients and their families become involved in their own health care. 

The Outreach Worker provides community outreach to farm workers and their families, primarily in their migrant labor camps, by actively reaching them to increase access to care and promote improved health outcomes.  Responsibilities include:

  • Engaging with migrant/seasonal farm workers and informing about InterCare services
    • Plans daily camp visits to designated service area
    • Initiates contact and distributes information to farm workers about InterCare programs and resources
    • Utilizes maps, patient charts and migrant camp directives to locate patient
    • Documents enabling services on logs, referral forms, and encounter forms
  • Provides appropriate referrals, support, and linkage to care
    • Maintains open and frequent communication with clinic staff and other outreach staff
    • Provides referrals in person, over the telephone, or any type of communication to outreach nurses, clinic staff or other community resources
    • Captures and relays pertinent information when making referrals
    • Identify strategies to facilitate a successful referral
  • Interpreting/translating medical information to patient
    • Communicating effectively medical terminology to migrant/seasonal farm worker
    • Interprets messages and relays information exactly as spoken to the migrant/seasonal farm worker
    • Possesses knowledge of content and structure of a foreign language (non-English) language including meaning
    • Uses alternative solutions to accommodate migrant/seasonal farm worker understanding

Experience and Skills:

General Development

  • BCLS certification
  • Ability to work effectively as a team
  • Ability to organize and manage time to maximize outreach efforts
  • Attends Migrant Resource Council meetings in assigned service area

Technical Skills

  • Proficient in Microsoft Office Suite
  • Ability to thoroughly document outreach activity on forms and in an Excel log

Communication Skills

  • Fluent in English and Spanish reading, writing, and speaking skills
  • Possesses a professional level of written and verbal communication skills
  • Ability to communicate complex concept in a clear effective manner
  • Possesses excellent cross cultural communication skills and the ability to communicate to clients & patients at all levels

Extensive travel; must have reliable transportation and able to work irregular and evening hours (approximately 25-30 hours per week).

Previous health education experience and knowledge of community resources desired.
